Medical Consent and Liability Waiver: I, the undersigned, wish to participate in a drop-in class offered by Ethos Jiu Jitsu. I understand that Jiu Jitsu is a martial art that involves physical exertion, close personal contact, and the risk of physical injury. I hereby consent to receive medical treatment deemed necessary in the event of an injury, accident, and/or illness during my participation. I agree to bear the expenses of such treatment and release Ethos Jiu Jitsu from any liability for such costs. Media Release: I permit Ethos Jiu Jitsu and its agents the right to take photographs and video recordings of me during the drop-in class and to use and publish these in print and/or electronically. I agree that Ethos Jiu Jitsu may use such photographs and videos for lawful purposes, including for example, such purposes as publicity, illustration, advertising, and Web content, without compensation to me. Acknowledgment of Risk and Waiver of Liability: I acknowledge the inherent risks associated with practicing Jiu Jitsu, including but not limited to bruises, sprains, broken bones, and more severe conditions. I fully accept and assume all such risks and all responsibility for losses, costs, and damages I incur as a result of my participation in the drop-in class. I understand the risks of physical injury inherent in Jiu Jitsu training and the potential for severe social and economic losses due to such injuries. Notwithstanding such risks, I agree to assume the responsibility for my safety and welfare while participating in the drop-in class, fully aware of my health and physical condition, and having discussed my participation with my physician, if necessary. I hereby release, discharge, and covenant not to sue Ethos Jiu Jitsu, its respective administrators, directors, agents, officers, volunteers, employees, and other participants, any sponsors, advertisers, and, if applicable, owners and lessors of premises on which the activity takes place, from all liability, claims, demands, losses, or damages on my account caused or alleged to be caused in whole or in part by the negligence of the "releasees" or otherwise, including negligent rescue operations; and I further agree that if, despite this release, waiver of liability, and assumption of risk, I, or anyone on my behalf, makes a claim against any of the releasees, I will indemnify, save, and hold harmless each of the releasees from any loss, liability, damage, or cost which may incur as the result of such claim. COVID-19 Acknowledgment: I acknowledge the ongoing COVID-19 pandemic and confirm that I am not participating in the drop-in class while under any health advisories that recommend isolation or quarantine. I understand the risks of exposure to COVID-19 and voluntarily agree to assume those risks as a condition of my participation in the drop-in class. I agree to follow all health and safety protocols that Ethos Jiu Jitsu has implemented. For Participants Under 18: This section confirms that if the participant is a minor, the parent or guardian has read and agreed to the terms of this waiver on their behalf.

I understand and agree that the terms of this waiver and release of liability shall apply to all future occasions on which I participate in any class, event, or activity at Ethos Jiu Jitsu, without the necessity of my signing an additional waiver for each session or event. I acknowledge that this agreement is intended to be as broad and inclusive as is permitted by the laws of the jurisdiction in which Ethos Jiu Jitsu operates and that if any portion thereof is held invalid, the balance shall, notwithstanding, continue in full legal force and effect. I affirm that my agreement to the terms herein is voluntary and further understand that to participate in any future class, event, or activity at Ethos Jiu Jitsu, I am not required to sign another document and that this waiver serves for all future participation. This agreement remains in effect until I provide Ethos Jiu Jitsu with written notification that I no longer wish to be bound by its terms or until Ethos Jiu Jitsu provides notice of change of terms. By signing below, I confirm that I have read and fully understand the terms of this waiver and release and voluntarily agree to its terms, acknowledging that this agreement shall bind the members of my family and spouse (if applicable), my heirs, assigns, and personal representative, and shall be deemed as a “Release, Waiver, Discharge, and Covenant Not to Sue” Ethos Jiu Jitsu.
